Fill Out Medical Questionnaire

Medical Questionnaire Must Be Complete and Answered honestly. Please note that if you answered yes to questions 3, 5, or 10, or to any of the questions on page 2., you would need to take all three pages of the medical questionnaire to your Medical Physician. A Medical Evaluation will be required. Your Physician will need to sign approving you safe to dive before any in water training can begin. Once form is completed, Please Email the form to bluedragonscuba@gmail.com

E - Learning is a Digital Learning Program that allows you to complete the Course Academics in the comfort of your home without the need of long classroom sessions and at your own pace.
